Application Procedures (只提供英文版)

 

1. As a smart hospital, CUHKMC encourages applicants to be environmentally friendly and send the completed application form together with the required documents as listed below to Human Resources Department via email to vms@cuhkmc.hk. In case of technical issue, applicants can still send the documents by mail to Human Resources Department, 12/F, CUHK Medical Centre, 9 Chak Cheung Street, Shatin, New Territories. 

  • Recent Photo (passport-sized) 
  • Application form on Scope of Clinical Practices (if applicable) 
  • Bank Account Instruction for Payment of Professional Fees 
  • Curriculum Vitae 
  • A reference letter from one of the medical doctors listed in Part VI of the application form 
  • Copies of diplomas and certificates for quotable qualifications 
  • Copy of Licence/Certificate of Registration from Medical Council 
  • Copy of Certificate of Specialist Registration (if applicable) 
  • Copy of valid Annual Practising Certificate 
  • Copy of other licence (e.g. Irradiating Apparatus Licence) 
  • Copy of medical indemnity insurance certificate/receipt. The coverage should commensurate with relevant specialty and scope of practices. 
  • Copy of Hong Kong Identity Card/Passport  
  • Name Card 

2. Administrative processing normally takes 4-6 weeks. A letter of notification will be sent to the Applicant by email, informing result of CUHKMC’s decision. 

3. Admission right and clinical privileges are granted on a 3-year basis and subject to renewal. 

4. Doctors are required to send the annual update of Practising Certificate and Medical Indemnity Insurance Certificate to CUHKMC. It is also required to inform CUHKMC in writing, whenever there is any change of personal data.
