CUHKMC “CUHK 60th Anniversary Alumni Privilege”

To celebrate the 60th Anniversary of The Chinese University of Hong Kong (“CUHK”), CUHK alumni and their families can enrol in selected health programmes of CUHK Medical Centre during the respective promotion periods of the three phases set out below for participation at the applicable Alumni Prices (the “Privilege”).

1The 2 or more persons (must include an alumnus) must enrol (whether at the same time or at different times) within the promotion period of the same Phase to enjoy the “CUHK Alumni and Families” Price applicable to such Phase. However, if such persons enrol at different times within the promotion period of the same Phase and the second person enrols after the first person has already paid for a programme of such Phase at the “CUHK Alumni” Price, the first person will not be entitled to refund of any payment in excess of the “CUHK Alumni and Families” Price applicable to such programme which has been paid for.
2An alumnus can also enjoy the “CUHK Alumni and Families” Price applicable to a Phase if he/she enrols in two or more programmes in respect of one Phase for himself/herself alone (whether at the same time or at different times) within the promotion period of such Phase. However, if such alumnus enrols in additional programme(s) of such Phase after he/she has already paid for a programme of such Phase at the “CUHK Alumni” Price, he/she will not be entitled to refund of any payment in excess of the “CUHK Alumni and Families” Price applicable to such first programme which has been paid for.

Terms and Conditions for the Privilege:

The Privilege is subject to the following terms and conditions (these “Terms and Conditions”): 

1.    The offer of the “CUHK Alumni” Price and the “CUHK Alumni and Families” Price for participation in the Programmes (as defined below) as set out above (“Privilege”) is only applicable to the alumni of The Chinese University of Hong Kong (“CUHK”) (“alumni”, each an “alumnus”) and members of their respective families (together with the alumni, each a “participant”) and is subject to these Terms and Conditions. Each participant is deemed to have read, accepted, and agreed to be bound by these Terms and Conditions upon enrolment in any Programme. 
2.    The Privilege is valid from 1 January 2023 to 31 December 2023 (both dates inclusive).
3.    The Privilege is only valid for the selected health programmes provided at CUHK Medical Centre in respect of each phase (“Phase”) as set out above (“Programmes”, each a “Programme”).
4.    To be eligible for the Privilege, in relation to any one Phase, each of the participants in relation to an alumnus (such alumnus is referred to as the “Relevant Alumnus”; any of such participants other the Relevant Alumnus is referred to as an “Other Participant”) must: -
(a)   enrol in (through completion and submission of the online form or via the CUHK Alumni Hotline as set out above) one or more Programme within the promotional period, and complete all services under the Programme(s) on or before the completion date, in respect of such Phase as set out below:

(b)    at the time of enrolment, provide information including, but not limited to, the full English name of the Relevant Alumnus, the identity of the participant in relation to the Relevant Alumnus (that is, being the Relevant Alumnus himself/herself or his/her family member), and the preferred Programme(s) for advance booking of the Programme(s); and 
(c)    at the time of registration at the first appointment of each Programme booked at CUHK Medical Centre, present to the staff the original, photocopy, or photo (digital or paper) of the valid Hang Seng The Chinese University of Hong Kong Alumni Platinum Card (principal card only), CUHK Library Card for alumni, Certificate of Graduation of CUHK, or CUHK Transcript of Academic Record (which must show the date of conferment of the relevant degree) of the Relevant Alumnus, or such other written evidence that CUHKMC (as defined below) accepts at its sole discretion, as proof of the alumni status of the Relevant Alumnus and, subject to clause 5 below,  pay for such Programme at the applicable “CUHK Alumni” Price or the “CUHK Alumni and Families” Price in full on the day of attending the first appointment. 
5(a)   To enjoy the “CUHK Alumni and Families” Price applicable to any one Phase, the Relevant Alumnus and the Other Participant(s) must enrol in the Programme(s) or the Relevant Alumnus must enrol in two or more Programmes for himself/herself alone (as the case may be) in respect of the same Phase (whether at the same time or at different times) within the promotion period of such Phase and complete all services under such Programmes on or before the applicable Completion Date, Provided that if such Other Participant(s) enrol(s) in the Programme(s) or the Relevant Alumnus enrols in additional Programme(s) (as the case may be) after the Relevant Alumnus has paid for a Programme in respect of the same Phase at the “CUHK Alumni” Price, the Relevant Alumnus will not be entitled to refund of any payment in excess of the “CUHK Alumni and Families” Price applicable to such Programme which has been paid for.
(b)    If subsequent to the Relevant Alumnus having paid for a Programme in respect of one Phase at the "CUHK Alumni and Families” Price:
    (i)    (in the case where the Relevant Alumnus and the Other Participant(s) have enrolled in the Programmes in respect of the same Phase) the Relevant Alumnus (if applicable) and such Other Participant(s) cancel(s) the appointment(s) for all such Programme(s) of the same Phase that has/have not been attended or fail(s) to attend any Programme of the same Phase; or 
    (ii)    (in the case where the Relevant Alumnus has enrolled in two or more Programmes for himself/herself alone in respect of the same Phase) the Relevant Alumnus cancels the appointment(s) for all such Programme(s) of the same Phase that has/have not been attended or fails to attend any additional Programme of the same Phase, 
    in each of the cases above, CUHKMC reserves the right to charge and collect from the Relevant Alumnus  the shortfall in the amount of the difference between the price paid and the “CUHK Alumni” Price applicable to the Programme which has already paid for.
(c)    The Relevant Alumnus must be one of the patrons for all the participants enrolled in relation to him/her to enjoy the Privilege. If subsequent to the Relevant Alumnus and the Other Participant(s) having enrolled in the Programmes in respect of the same Phase, the Relevant Alumnus cancels the appointment(s) for all the Programme(s) enrolled for himself/herself or fails to attend any Programme of such Phase, CUHKMC shall be entitled to charge the Other Participant(s) at the prevailing listed price of the Programme(s) on the day of attending the first appointment or, in the event that any Other Participant has paid for the Programmes at the “CUHK Alumni and Families” Price, CUHKMC reserves the right to charge and collect from such Other Participant the shortfall in the amount of the difference between the price paid and the prevailing listed price of the Programme(s) on the day of attending the first appointment.
(d)    In the event that the proof of alumni status referred to in clause 4(c) above of a Relevant Alumnus has been obtained by fraud, is forged or is otherwise invalid, or such proof or the alumni status of such Relevant Alumnus has been suspended, cancelled, or invalidated (as applicable) for whatever reason at the time of attending the Programme(s) by a participant in relation such Relevant Alumnus, all participants in relation such Relevant Alumnus will be disqualified from enjoying the Privilege from the time of such invalidity, suspension, cancellation, or invalidation (as applicable) without prior notice, and CUHKMC shall be entitled to charge each of such participants at the prevailing listed price of the Programme(s) on the day of attending the first appointment; or, if any of such participants has paid for the Programme(s) at a price under the Privilege, CUHKMC reserves the right to charge and collect from such participant the shortfall in the amount of the difference between the price paid and the prevailing listed price of the Programme(s) on the day of attending the first appointment.  
6.    The Privilege is only applicable to persons aged 18 or above.
7.    Availability of the services to be provided under the Privilege will be subject to the service capacities of CUHK Medical Centre and each participant’s suitability to receive the services to be provided under any Programme enrolled as determined by the relevant medical practitioner of CUHK Medical Centre at his/her sole discretion. If such medical practitioner determines that a participant is not suitable to receive the services provided under any Programme enrolled, CUHK Medical Centre will only charge for the doctor consultation fee, and any related diagnostic and medication fee (if applicable). In such event, if (in the case where there are two or more participants in relation to the same Relevant Alumnus enrolled for the Programme(s) in respect of one Phase) the aforementioned participant is the only participant that causes the other participant(s) to be eligible for the applicable price in respect of such Phase under the Privilege or (in the case where the Relevant Alumnus is the only participant in relation to himself/herself in respect of one Phase) the Relevant Alumnus has only enrolled for two Programmes in respect of such Phase, such other participant(s)’ eligibility or the Relevant Alumnus’ eligibility (as the case may be) to enjoy the applicable price in respect of such Phase under the Privilege will not be affected.  
8.    The prices under the Privilege do not include fees and/or charges for any treatment or medication.
9.    The Privilege cannot be used in conjunction with other offers or discounts.
10.    The Privilege cannot be transferred or redeemed or exchanged for cash or other products or services, and the fees paid for the services under the Privilege are not refundable and cannot be exchanged for other products or services. 
11.    Before receiving the services under the Privilege or other services at CUHK Medical Centre, the participant has to accept its relevant service terms and conditions.
12.    CUHK Medical Centre Limited (“CUHKMC”) reserves the sole right to vary or terminate these Terms and Conditions or any aspect of the Privilege at any time without prior notice.
13.    These Terms and Conditions shall be governed by, and construed in accordance with, the laws of Hong Kong.
14.    No person other than the Relevant Alumnus and CUHKMC will have any right under the Contracts (Rights of Third Parties) Ordinance (Chapter 623 of the Laws of Hong Kong) to enforce any provision of these Terms and Conditions.
15.    In case of any disputes in relation to any aspect of the Privilege, the decision of CUHKMC shall be final and conclusive. 
16.    In the event of inconsistency between the English and the Chinese versions of these Terms and Conditions, the English version shall prevail.
