Diabetes Risk Assessment Programme

One in every 10 people in Hong Kong has diabetes*. If not properly controlled, it will lead to chronic diabetic complications with serious consequences. Persistently high blood glucose levels destroy blood vessels and nerves, causing damage to multiple organs such as the eyes, brain, heart, kidneys, feet and more. People diagnosed with diabetes for 10 years or more have increased chance of dying from cancer especially in the presence of poor blood glucose control and other risk factors^.

Our Diabetes Risk Assessment Programme (the Programme) is fully supported by the Endocrinology & Diabetes Team of the Chinese University of Hong Kong (CUHK) and follows international guidelines where we use individualised data collected during structured assessment to issue a personalised report. The report includes risk categorisation, future risk of complications and personalised advice on medical therapies and self-management for people with diabetes through the evidence-based Joint Asia Diabetes Evaluation Program (JADE®) platform. The report also includes written recommendations by Specialists in Endocrinology, Diabetes & Metabolism and the report will be explained by experienced nurses, aimed at reducing the risks of hospitalisation and complications in people with diabetes.
