Terms and Conditions for Non-Emergency Medical Transfer
- The non-emergency medical transfer service (the “Service”) is delivered by Amber Medical Group Limited (“Amber Medical”), a private ambulance service provider contracted to CUHK Medical Centre (“CUHKMC”). CUHKMC is only responsible for booking and arranging the Service.
- All bookings for the Service must be made in advance with CUHKMC and are subject to its availability and clinical judgment of its suitability for the patient. CUHKMC will provide fee quotes for the Service requested. Booking is not confirmed until patient is notified by CUHKMC of acceptance of booking by both CUHKMC and Amber Medical.
- CUHKMC reserves the right to not accept any request for booking for the Service, and to postpone, or cancel a transfer if deemed unsafe or clinically unsuitable notwithstanding any confirmed booking.
- Fees quoted must be settled in full before commencement of the Service. All prepaid fees are non-refundable, non-transferable, and not exchangeable for other services or products. The Service is not applicable for direct billing.
- Additional charges may apply for different circumstances, for example, extended service distance and/or time, outside office hours service, additional medical equipment needs, etc.
- A charge of the total quoted fee will apply to any cancellation or rescheduling request received by CUHKMC less than 4 hours prior to the scheduled appointment time.
- The patient must accept all the inherent risks and limitations associated with the use of the Service.
- In the event an emergency situation arises during transit, the patient may be transferred to the nearest appropriate medical facility.
- Personal data of the patient and/or patient’s contact person(s) required for the provision of the Service may be shared with Amber Medical by CUHKMC.
- CUHKMC reserves the right to amend these terms & conditions without prior notice. In the event of any dispute, the decision of CUHKMC shall be final.
