Lee Heng Charity Foundation Cognitive Health Assessment Subsidy Scheme
Thanks to the generous contribution from the Lee Heng Charity Foundation, individuals who meet the eligibility criteria of this ‘Cognitive Health Assessment Subsidy Scheme’ (Subsidy Scheme) can apply for financial support for conducting designated cognitive health assessment and investigation items that are considered as appropriate for and recommended to such individuals by our neurologists according to their clinical needs.
The selection of designated cognitive health assessment and investigation items as supported by the Subsidy Scheme (collectively, Investigation Items) is designed by neurologists under our Neurocognitive Centre (Neurocognitive Centre). The Investigation Items include doctor’s consultation by neurologist, the Montreal Cognitive Assessment 5-Minute Protocol Hong Kong Version (HK-MoCA 5-Min Protocol), blood-based biomarker testing, and AccuBrain® AI Brain Image Analysis. Based on participants' clinical needs, the advanced blood-based biomarker testing, supported by AccuBrain® AI Brain Image Analysis, is more accurate and effective in detecting early-stage Alzheimer's Disease and other neurodegenerative cognitive disorders.

Subsidy Scheme Flowchart
Before application
- Individuals interested in the 'Cognitive Health Assessment Subsidy Scheme' must contact our Neurocognitive Centre (NC) before submitting application, either by phone at 6629 2260 or in person
- NC nurse will enquire applicant to provisionally confirm if applicant meets the major eligibility criteria of the subsidy scheme, including being a valid Hong Kong ID card holder and having a family history of dementia / other neurodegenerative cognitive disorders or exhibiting symptoms of potential dementia / other neurodegenerative cognitive disorders
Submission of application
- Applicant submits duly completed and signed application form and relevant documents to NC in person or by post
- Hospital assesses the application, and then approves the application if all eligibility criteria are met and grants the relevant tier of subsidy*
- NC nurse informs applicant of the application result and schedules an appointment for successful applicant (participant)
*Whether subsidy application is approved is also dependent on whether the amount of donation designated for this subsidy scheme has been depleted at the time of assessing the application.
Consultation and investigation
- NC nurse first administers the HK-MoCA 5-Min Protocol to participant
- Participant then attends doctor consultation by neurologist, and neurologist may recommend investigations including blood-based biomarker testing and/or Accubrain® AI Brain Image Analysis (Brain Image Analysis) according to participant's clinical needs
- Blood test and MRI for Brain Image Analysis would be tentatively scheduled for the same day if both are recommended by neurologist at the same time. If Brain Image Analysis will only be arranged after review of blood test results by neurologist, NC nurse will contact participant after such review for scheduling the MRI appointment. However, MRI appointment is subject to service availability of the imaging centre
After consultation and investigation
- Blood test results will be available within 4–6 weeks after the blood sample is collected
- Accubrain® Brain Image Analysis Report will be typically available within 5–7 working days after performing MRI
- Neurologist reviews the investigation results
Debriefing
- NC nurse explains the investigation results to participant
- If follow up or treatment is required and participant wishes to have follow up or treatment in CUHK Medical Centre, NC nurse will schedule a follow-up appointment for participant
- If participant wishes to have follow up or treatment in HA Hospitals, neurologist will issue referral letter for participant
Terms and Conditions for the
‘Cognitive Health Assessment Subsidy Scheme’
- (a) These terms and conditions are applicable to the applicants (each an Applicant) for the subsidy (Subsidy) provided under the ‘Cognitive Health Assessment Subsidy Scheme’ (Subsidy Scheme). Once the subsidy application is submitted, the Applicant is deemed to have read, understood, and accepted these terms and conditions, all the terms and conditions set out in the application form, and all other rules as stipulated by CUHK Medical Centre Limited (CUHKMC) for the Subsidy Scheme, and agreed to be bound by all of the foregoing.
(b) For an Applicant who has been approved of the Subsidy participates in the cognitive health assessment and investigation items designated to be supported under the Subsidy Scheme (collectively, Investigation Items), if the Prevailing Price (as defined in clause 3 below) on which the Subsidy amount is calculated is an offer price, any term that imposes time limit on receiving services under the related offer (if any) as set out in the terms and conditions for such offer shall not apply to such Applicant.
(c) The Investigation Items include initial consultation by neurologist^ (including clinical assessment by the neurologist), the
Montreal Cognitive Assessment 5-Minute Protocol Hong Kong Version (HK-MoCA 5-Min Protocol) conducted by medical staff, and other Investigation Items (which include designated blood-based biomarker testing and/or AccuBrain® AI Brain Image Analysis) as the neurologist considers as appropriate for and recommends to the Applicant. Applicants are required to contact the Neurocognitive Centre of CUHK Medical Centre (Neurocognitive Centre) to enquire about the details of the Investigation Items, the fee of each Investigation Item, and the calculation of Subsidy before submitting their applications.
^ This is provided by neurologist designated by CUHKMC.
- Eligibility:
To be eligible for the Subsidy, an Applicant must meet the following eligibility criteria and submit application in accordance with the prescribed procedure:
(i) Applicant must be a holder of a valid Hong Kong Identity Card;
(ii) Applicant has a family history of dementia or other neurodegenerative cognitive disorders, or exhibits symptoms of potential dementia or other neurodegenerative cognitive disorders, and Applicant must contact the Neurocognitive Centre by phone or in person before submitting the application for our medical staff to enquire the Applicant to provisionally confirm that the Applicant fulfills criteria (i) and (ii);
(iii) Applicant must not possess any medical insurance coverage or be the beneficiary of any medical insurance or coverage (excluding employee medical insurance coverage provided by employers); and
(iv) Applicant has to meet one of the financial requirements set out in clause 3 below. - Subsidy by Tiers and Financial Requirements:
The levels of the Subsidy are categorised by tiers according to the financial requirements below met by the Applicant:Tier of Subsidy Financial

(b) Average monthly total household savings in the past 12 months: under HK$1,500,00030% of the fee ^^ The Subsidy amount is calculated based on the Prevailing Price of the relevant Investigation Items on the date of application submission. For the purpose of the Subsidy Scheme, the ‘date of application submission’ means the date of receipt of application by the Neurocognitive Centre if submitting in person or the date of the post office stamp if submitting by post; and ‘Prevailing Price’ of an Investigation Item on the date of application submission means the lower of (i) the original fee of such item that is valid on the date of application submission or (ii) the offer price of such item under any valid offer published on the website of CUHK Medical Centre (‘Relevant Offer’), or other offer price of / discount on such item that is applicable to the Applicant (‘Applicable Offer/Discount’), on the date of application submission, provided that item (ii) is only applicable when the Applicant fulfills all the terms and conditions as required for enjoying the Relevant Offer or those for enjoying the Applicable Offer/Discount, and the Applicant cannot enjoy the Relevant Offer in conjunction with the Applicable Offer/Discount or other offers.
- Application Procedure and Approval:
Applicant must submit a duly completed and signed prescribed application form and provide documentation for supporting the financial requirements and all other information and documentation as requested by CUHKMC in accordance with the application requirements of CUHKMC. CUHKMC will assess and verify the eligibility for the Subsidy of the Applicant based on the information and documentation provided by the Applicant. The decision of CUHKMC made on the applications for Subsidy shall be final and conclusive. - Use of Subsidy:
The Subsidy can only be used to pay for/deduct from the fee(s) (in full or in part) of the relevant Investigation Item(s) paid on the day of receiving such item(s). Any medical consultation, treatment, procedure, investigation, medication, or service provided beyond the scope of the Investigation Items recommended by the neurologist shall be at the Applicant’s own costs. - Due to the nature of the Subsidy Scheme, once the amount of donation designated for the Subsidy Scheme has been depleted, CUHKMC will stop providing any Subsidy and the Subsidy Scheme will terminate without prior notice; that is, even if an application is submitted, the Applicant may not receive the Subsidy and has to pay the full fee amount for the relevant service at the Prevailing Price on the date of receiving the service.
- The Subsidy is not transferable and cannot be exchanged for cash or other products or services. Any fee self-paid by an Applicant under the Subsidy Scheme is not refundable and cannot be exchanged for other products or services.
- CUHKMC reserves the right to amend these terms and conditions at any time without prior notice.
- In case of any dispute, the decision of CUHKMC shall be final and conclusive.
- These terms and conditions shall be governed by, and construed in accordance with, the laws of the Hong Kong Special Administrative Region. No person other than the Applicants and CUHKMC will have any right under the Contracts (Rights of Third Parties) Ordinance (Cap. 623) to enforce any of these terms and conditions.
- In the event of inconsistency between the English and the Chinese versions of these terms and conditions, the English version shall prevail.
